I can not locate the spreadsheet with documents uploaded through my forum. When I click view spreadsheet on my forum editor page nothing happens.

You’re talking the Google Sheets integration in your Form Builder widget, right?
If this is the case, could you please specify whether only the files uploaded are not displayed in the spreadsheet or all field inputs are missing? A screenshot of your spreadsheet would be much appreciated

@Max I can not find the spreadsheet at all. It is not in my sheets and when I click view sheets through the form builder nothing happens
