Create a Checklist form in an accordion format. Each Topic to have their own field for tasks that we can edit/add as the accordion Topic expands. Each topic should have 3 boxes to chose from; 1) Yes 2) No 3) N/A and then a button for ‘Take Photo’ and a button for ‘Notes’ after each task.
At the bottom would be a Submit button. After completion, all the answers populate to a Spreadsheet showing each answer to the tasks with any photo and/or Note taken for that task.

Could you please describe how the Take a Photo and Notes buttons should work? Take a Photo button lets users upload images to the form and Notes field let users add some comments which will show up in the spreadsheet, am I right?

Yes, that is correct. The photos would also need to populate to the spreadsheet as well as any notes taken.

In this case our Form Builder should be just the thing for you. Here you should choose a Multistep layout to separate each topic. With the Choice field you can show a question with multiple answer options (Yes, No, N/A).
Long Text field can be used for adding notes and File - for the photo uploading:
To make the results appear in the spreadsheet, you should connect your widget to Google Sheets on the Integrations tab:
