Subscription form not showing all boxes

I’ve built a subscription form with three text boxes for names and email plus two choice option boxes and a consent box but only the three text boxes are showing when I copy in the code?

We’ve fixed the issue by adding this code to the Custom CSS field on the Appearance tab of your widget’s settings:

label {
  display: flex !important;
}

Please check your website and let me know if it’s fine now :slightly_smiling_face:
