Social media bar, with Icons and the counter of followers

We need a Social Media Bar, to insert in our Site that’s connected to each Social Media and updates automatically the number of followers of each SM.
Justa a Simple bar with Icons and the number of followers below each icon.
And the possibility of been link each icon to it social media page.
