Header title: add parameter to display the title of the page the form is displayed on (dynamically)

Add a way that the Form Heading can have a code to insert the Page/Post Title in it.

So the header title of the form could be of type: “Appointment Request Form for [Page-Title]”, where [Page-Title] will change dynamically, depending on the page the form is installed on.

That way it would make the form custom to that particular page.
