Hi, I need a form that includes social media icon to ask the visitors to follow us on other social media platforms

You can do this using the Form block in our Banner or Popup apps, but keep in mind that they have a limited number of fields comparing to Form Builder and Contact Form apps:
So, you just need to add the required form fields to the widget. After that, you should add an HTML block and paste the installation code of your Social Media Icons widget there:
Note: In Banner, Social Media Icons widget won’t be displayed in the configurator, but will appear on the published widget’s version on your website.
