Configuring Contact Form to integrate with LeadsEngine CRM

We utilize ElfSight on our MoxiWorks Website. Our site is c21circle.com. We have created various contact forms and would like to have them integrated with LeadEngine - the CRM behind MoxiWorks.

Here is the information provided by LeadsEngine. Is this possible?
How To Configure New Email Source

Ask your developer to configure the form according to the following specifications:

· Emails from form should be sent to: ######.lead@leadsengine.realogy.com

Subject line should be: ,
Email or phone field are required
· TO: ######.lead@leadsengine.realogy.com

· SUBJECT: ,

Include below within the email body:

· FIRST_NAME: John

· LAST_NAME: Doe

· EMAIL: user@domain.com

· PHONE: #-###-###-####

· MLS_ID: #########

· PROPERTY_ADDRESS: 12345 Main Street

· PROPERTY_CITY: City

· PROPERTY_STATE: State

· ZIP_CODE: #####

· PROPERTY_TYPE: Single Family Residence

· COMMENT: Comments here
