I’m afraid such integration is not supported in our Form Builder widget, but I think an Appointment Booking widget seems to align with your idea, right?
We do have this widget in our Wishlist, and we’re also planning on releasing it this year. You can vote for it to get a notification upon its release here: Appointment Booking
