add a Conversational Forms survey with multiple page image question with name and info and a contact form would be awsome.

You know, we have a Survey template in our [Form Builder](https://elfsight.com/online-form-builder/) widget, could it be a solution for you? It has a Multistep Form option, so you can easily customize it the way you need 🙂
